Public Health Nursing Program

Our Public Health Nurses (PHNs) allow the continuum of care to reach some of the most at-risk members of our community. PHNs are contracted through the joint Boards of Health to

• Conduct well baby visits
• Run chronic disease self-management clinics (for conditions like hypertension, diabetes, cardiovascular disease)
• Disseminating public health information at community events and at key points throughout the year
(back-to-school, flu season, etc.)
• Assist with vaccination clinics and the Island’s annual flu clinic
• Host visiting hours at town libraries for the Island’s homeless population
• Visit elders at the Councils on Aging and in their homes for wellness checks and meal deliveries
• Work with community members on nutrition goals, understanding nutrition guidance, and at-home culinary guidance
